Dr. Parks, with the assistance of Dr. Hopper will be discussing the often hidden, but strongly correlated connection between physical and mental conditions,particularly as they become chronic and more difficult
to manage. The ultimate goal of the program is to equip future providers with understanding of the disparities in resources, proximal care, and convenient follow up that clearly accelerate and worsen chronic condition management.
A two-way negative cycle of depression, anxiety, and other more severe mental conditions feeds the cycle of worsening medical status. Both are intertwined, and efforts “as-quickly-as recognized” to interrupt both the
medical and behavioral cycles improves medical, behavioral, and cost outcomes (reduces cost).
The audience will be able to:
• Describe the correlation of mental health diagnoses and
chronic medical conditions
• Explain how urgent, unexpected medical healthcare
utilization relates to comorbid mental conditions
• Describe and discuss 2 results-producing care models
designed to address the interrelation between medical
and behavioral conditions
• Describe both the gap and a potential solution to improve
“community tenure” when at-risk patients leave a hospital
treatment episode
